1. I have read and I understand the complete description of your program.
2. I understand that I am receiving the 4 items listed in the “IMO/AGENCY” Package and not just the 20 weekly “RESPONSE” leads.
3. I understand I will receive online access to the 4 items listed in the "IMO/AGENCY" Package within one business day.
4. I understand I will receive the "Response" leads (20 per week for up to 12 weeks based on the amount of carriers I selected above) via EMAIL beginning in
approximately 7-10 BUSINESS DAYS. I understand I must be eligible to contract with or transfer with the carrier for it to count towards the free leads.
5. I understand that I will receive the leads that I selected on my sign up form and that
I cannot change my order once I start receiving my free leads .
6. I understand that I should follow the instructions provided with the leads in order to generate the maximum amount of appointments.
7. I understand that it is my responsibility to notify “National Annuity and Life Sales” within 10 business days if my leads are not received and that “National Annuity
and Life Sales” is not responsible for my leads being deleted or placed in a spam folder by my email service provider.
8. I understand that the 20 weekly “RESPONSE” leads are just one part of the “IMO/AGENCY” Package and are meant to be a supplement to the other items in the “IMO/AGENCY”
Package.
9. I understand I should implement some of the other 10 lead programs to take full advantage of your program along with qualifying for your “First App” Promo.
10. NALS is making an initial investment in your business by providing you with our leads along with access to our exclusive copyrighted material, training and lead
programs. Therefore we expect an honest effort on your part to work the leads and programs as instructed and attempt to place a portion of your business with
the carrier(s) you contracted with through NALS. For our protection and to prevent abuse of our leads and programs, NALS does not provide contract releases within the first six
months of joining our program.
11. I understand this program is for my use only and may not be copied or distributed to any other persons.
12. I understand that the “up to 12 weeks of leads” contracting offer is ONLY good at time of sign up and that if I decide to add carriers to NALS at a later date that it DOES NOT add extra weeks of leads to the offer.
13. I understand that the 20 weekly leads, direct mail leads and telemarketing leads are all provided by a 3rd party insurance lead vendor and not generated or provided directly by NALS. NALS assumes no responsibility for the accuracy or quality of the leads.
14. I understand that certain rules, limits, exclusions and restrictions may apply with this offer.
I understand that National Annuity and Life Sales will provide the following services to me:
1. They will set up and maintain my Personalized IMO/AGENCY Website to recruit and contract agents ON-LINE.
2. They will handle and process ALL of my agents contracting paperwork that I submit.
3. They will provide EACH OF my AGENTS that I contract The “GENERAL AGENT” Package at no cost unless they are declined contracting.
4. Upon my agents first submitted application with my organization, they will make available to my agents the “First App” Promo.
5 They will provide toll-free support for me and my agents.
6. Provide up to 1000 names per month of insurance agents to contact as requested.
I understand that BEFORE I am provided with the above services I MUST:
1. Execute a new or transfer an existing contract with at least one of your listed insurance carriers AND pay a $99 annual website fee before receiving these 4 items. You can cancel the website anytime after the first year.
I understand and agree to the following:
1. I must submit my agents contracts directly to National Annuity and Life Sales for processing.
2. Once I submit my $99 payment, it is NON-REFUNDABLE unless my contracting is declined with ALL of the carriers I submit. NO exceptions.
3. That if I am declined contracting or not approved with at least one of your carriers, I will also not receive the 20 weekly leads.
4. National Annuity and Life Sales is providing me with this opportunity and will provide assistance, but has not guaranteed that I will be successful or make any stated amount of money and that my success and income will be determined by my effort.
5. That I am responsible for any cost and effort in recruiting my agents.
6. I must obtain a non-residence license in any state that I recruit agents in outside my resident state unless not required by that state.
7. I am an independent contractor and that my commissions and agent overrides are paid directly from the insurance carriers unless otherwise stated. All Commissions are paid upon issue and delivery of policy, not submitted.
8. Any carrier appointment that I transfer AWAY from National Annuity and Life Sales will be removed from my IMO/Agency website and any agents recruited with the carrier will no longer qualify for the programs offered by "National Annuity and Life Sales".
9 I understand that my IMO/AGENCY website will ONLY include the carriers that I am licensed with through National Annuity and Life Sales.
10. After I submit my application and payment, I must sign then fax or mail the signature pages to National Annuity and Life Sales to complete the contracting process.

That the “First APP” Promo becomes available upon my first submitted case. For carriers that do not process agent contracts UNTIL the first submitted case, then I understand that I MUST be APPROVED by that carrier before the case is considered submitted. I also understand that an incomplete app or an app that has any missing documents will NOT be considered submitted UNTIL all missing documents are received from the agent.

The “First APP” promo or any other program, incentive or promotion offered by “National Annuity and Life Sales” is only valid to our agents in good standing. (No current Letter of Intent to transfer or agent debt with us).
